The Value of Self-Management
Diabetes is a progressive, chronic disease that must be controlled for life. The good news is 90 to 95 percent of the process of controlling diabetes belongs to the person with diabetes. Diet and physical activity are just as important as medications and monitoring sugar levels in controlling the disease.
Hackley Hospital's Self-Management Program is designed to help people with diabetes work with their doctors to control diabetes. For many people, taking such an active role in their own healthcare is a real change in mindset. Because it can represent such a change, our program helps people adjust their diabetes to their lives, instead of adjusting their lives to their diabetes. Hackley's diabetes program is recognized by the American Diabetes Association and is certified through the Michigan Department of Public Health. It is the only such program in Muskegon County.
Getting With the Program
The Hackley Hospital Diabetes Self-Management Program consists of seven vists. The first two visits are with a registered nurse and a dietitian. Next, participants attend two four-hour group classes. After practicing what they've learned for a month, the person with diabetes returns for a follow-up visit to make sure their program is working for them. During these visits, participants are taught how to control their diabetes by managing diet, medications, physical activity, glucose, and the prevention of complications.
Class Reunion
After completion of the Program, the door is left open for all people with diabetes to come back with any questions, or to seek advice. An annual follow-up is also scheduled to help track diets, medications and exercise programs.

In order to participate in the Hackley Hospital Diabetes Self-Management Program, you need a referral from your doctor; however, most doctors are delighted to have a person with diabetes involved in their own care.
