Stroke Care Team
The Hackley Hospital Stroke Care Team specializes in the diagnosis and treatment of stroke and stroke-related injuries. Every member of our multidisciplinary team - paramedics, physicians, nurses, technicians, therapists, social workers and chaplains understands the importance of stroke prevention, early diagnosis, treatment and follow-up care.
Our goal is to prevent strokes whenever possible through education, and to provide complete and total care for those who have suffered a stroke.
What you can Expect
Your doctor will provide the necessary and appropriate medical care while you are in the hospital. In addition, your treatment starts with a thorough evaluation to identify your strengths and weaknesses, both before and after the stroke. Once these are determined, a special program will be developed for you to meet your individual needs. This program will change as you progress. The team that will evaluate and treat you is composed of:
¹ Your doctor and nurse
¹ Occupational therapists
¹ Physical therapist
¹ Speech therapists
¹ Social workers
¹ Dietitians
Your special program
Your program may include some of the following specialized treatments or services for:
¹ Strengthening exercises to improve walking, sitting, standing and balance
¹ Relearning eating, safe swallowing, dressing and personal grooming
¹ Relearning memory, speech and problem-solving
¹ Determining needs for devices such as wheelchairs, walkers, splints and or braces
¹ Living at home safely
Adjusting to life outside the hospital
When it is time for you to leave, you and your family will be offered choices for continued care depending on your needs such as:
¹ Outpatient therapies
¹ Therapies in your home done by Visiting Nurses
¹ Transfer to Rehabilitation Unit for continued therapy
¹ Referral to a long term acute care hospital for further treatment and rehabilitation
¹ Referral to a nursing home for rehabilitation
