A Patient's Guide to Insurance Information:
Insurance policies and payment procedures can be complex and confusing in today's healthcare environment. The following information will help you understand what to expect from the bill you receive from Hackley Hospital and other healthcare providers. If you have questions about specific coverage or non-covered charges, please contact your insurance carrier for an explanation of your insurance coverage.

Hackley Hospital will bill both your primary and secondary insurance, so it is important for you to tell us about all your insurance plans, especially if both spouses are working. Hackley Hospital may verify the insurance information that you provide us. If you are a Medicare patient, please be sure to mention your supplemental (medi-gap) policies.

Co-pay and deposits may be required upon admission. You will be notified of any deposit requirements as soon as your insurance coverage has been verified by our office. For your convenience, we accept MasterCard and Visa.

What Your Bill From Hackley Hospital Will Include:
If you were an inpatient, your bill will include charges for hospital tests and services, room accommodations, supplies and drugs. Outpatient charges may include emergency room visits, clinic or therapy visits, observation beds, tests, other services (including lab specimens sent to us by your physician's office) and supplies.

Medicare Supplemental Insurance:
We will bill your supplemental insurance carrier for any portion of the bill that Medicare does not pay. If your supplemental insurance does not pay within 45 days of our billing, we will send you a statement showing the balance due. It is your responsibility to follow up with your insurance company to find out why they failed to make payment.

Medicaid:
If you have active Medicaid coverage, we will bill the carrier for you. If you have any other insurance, Medicaid will not pay until the other insurer has paid or denied payment. IT IS IMPORTANT THAT YOU PROVIDE US WITH ALL OF YOUR INSURANCE INFORMATION.

A Family Independence Agency (FIA) caseworker is located in the Patient Financial Services Department at Hackley Hospital to take your application and provide assistance for Muskegon County as well as other counties. You can call the FIA caseworker at 231.728.4076 to make an appointment.

If you are denied Medicaid coverage, Hackley Hospital uses the Services of Patient Resource Consultants (PRC) to review the denial. This company makes sure that your application was completed correctly and that the Family Independence Agency (FIA) determination was appropriate. You may be asked to sign a form, which authorizes PRC to represent you in your attempt to obtain Medicaid coverage. This service is free to our patients.

Blue Cross:
We will bill your Blue Cross plan for you. Each Blue Cross plan has its own coverage provisions. Your plan may require you to obtain prior authorization for treatment. Some plans also require you to pay deductible and co-insurance amounts. Any unpaid balance after Blue Cross pays will be your responsibility. If you feel Blue Cross has made an error in paying your account, you should contact them directly. You will receive a statement from Hackley Hospital only after Blue Cross pays or denies payment.

Other Insurance:
Hackley Hospital also bills for all individual, group, self-insured, HMO, PPO and Champus policies. It is your responsibility to obtain prior authorizations for services if it is a requirement of your policy. It is also your responsibility to obtain and complete any required forms. You will not receive a bill from Hackley Hospital unless your insurance does not pay in full. Any deductibles, co-payments and non-covered charges are your responsibility.

Auto Accident:
If you require the service of Hackley Hospital as a result of an auto injury, please provide us with your auto insurance information in addition to your health insurance. You are responsible for filing an accident claim with your auto insurance. Any balance remaining after insurance is your responsibility to pay within 30 days.

Workers' Compensation:
If you are injured at work, Hackley Hospital must bill Workers' Compensation. We will send the bill to your employer or employer's insurance company. It is your responsibility to make sure that an accident claim and the appropriate Workers' Compensation papers are completed by the employer to ensure prompt payment. You will not receive a bill from Hackley Hospital unless Workers' Compensation rejects your claim or your Workers' Compensation carrier does not pay within 60 days.

Cashier:
The Hackley Hospital Cashier is located on the main floor. From the Main Entrance go past the information desk and head right down the hall. Pass the Laboratory and Radiology Departments, the Cashier is on the right hand side across from the Radiology Department.

Cashier's hours are 8:00 a.m. to 4:30 p.m. Monday through Friday. The cashier can also take credit card payments over the phone.

Financial Counseling:
Hackley Hospital will provide financial counseling if you request it. You may also apply for financial assistance through our financial counselor. If you are unable to pay the balance due from your hospital stay Hackley Hospital offers no-interest, short term payment plans based on your balance due.
