Patient Rights and Responsibilities
Hackley Hospital, our employees and medical staff, is committed to providing healthcare that respects the rights of patients and empowers patients to assume responsibility for their health. Access to Care/Discrimination:
Patients will be treated fairly in regards to medical treatment and facilities based on availability and medical condition regardless of race, sex, national origin, handicap, age, HIV status, marital status, sexual/gender orientation, religious beliefs, or sources of payment for care.
Respect and Dignity:
Patients have the right to considerate, respectful care at all times.
Privacy and Confidentiality:
Patients have the right to personal privacy including privacy of personal medical information.
Pain Management:
Patients have the right to expect continuous assessment of their pain and a process of mutual planning to address the management of pain.
Personal Safety:
Patients have the right to expect care and treatment to be provided in a safe, secure and functional environment. Hackley Hospital and its associates have a strong commitment to safety.
Health Information:
Patients have the right to, and are encouraged to receive from doctors and other direct caregivers, current and understandable information concerning diagnosis, treatment and prognosis. Patients have the right to discuss and request information related to the specific procedures and/or treatments, the risks involved, the possible length of recuperation, and the medically reasonable alternatives and their risks and benefits. Patients have the right to review their medical record, preferably with their physician. Patients also have the right to receive, for a reasonable fee, a copy of their own medical record upon request. Call (231) 728-4920 after discharge and they will discuss with you.
Communication:
Patients who do not speak or understand English, or are hearing impaired, will have access to translation and interpretation services necessary to meet medical needs. Patients will routinely be informed of all treatment outcomes. Ask your nurse or contact an interpreter.
Decision Making/Advance Medical Directives:
Patients have the right to make healthcare decisions prior to the delivery of healthcare and to make this known to the care providers via the Advance Medical Directives. Patients have the right to make decisions involving their healthcare during delivery of care. Patients have the right to seek advice or a consultation with the Ethics Committee. Patients have the right to accept medical care or to refuse treatment to the extent permitted by law, and to be informed of the medical consequences or results of their decisions. Research:
Patients are entitled to information concerning the experimental nature of procedures proposed as a part of their care, and shall have the right to refuse to participate in the experiment without compromising their continued care.
Transfer and Continuity of Care:
Patients have the right to expect continuity of care and to be informed by doctors and other caregivers of available and realistic patient care options when hospital care is no longer appropriate or available at Hackley Hospital. Patients will not be transferred unless the institution has accepted them for transfer. Patients must also have the benefit of complete information and explanation concerning the need for, risks, benefits, and alternatives to such a transfer. Social work services are available 24 hours a day, 7 days a week. Ask your nurse for assistance.
Patient Complaints:
Patients have the right to be informed of available resources for filing complaints, resolving disputes, arguments, grievances and conflicts. Ask to speak with the Nursing Supervisor.
Charges:
Patients have the right to be informed of charges for services, payment methods, financial assistance and payment options.
Spiritual/Pastoral Care:
Patients are entitled to spiritual/pastoral care as desired from their own pastor, priest, rabbi or other spiritual care provider or religious group, unless it interferes with their medical care or that of others, or is disruptive. Spiritual/pastoral care may also be accessed from chaplains serving Hackley Hospital as part of Pastoral Care Services.
End-of-Life Care:
Patients have the right to information about end-of-life treatment options, in respect to personal dignity, and their spiritual and cultural beliefs, while promoting comfort.
Family/Significant Other Participation:
Patients have the right to include or exclude any or all family members, or significant others, in the participation of their care.
Patients are Responsible for:
- Following Hackley Hospital's rules and regulations affecting patient care and conduct to promote quality care and a safe environment.
- Providing a complete, accurate and honest medical history and information.
- Making it known whether they understand a suggested course of action and the things they are expected to do. Ask questions.
- Following the recommendations, advice, and treatment course prescribed by the doctor and willingly agreed upon by the patient.
- Providing information about unexpected complications or problems that arise in an expected course of treatment.
- Conducting themselves in a manner that respects the rights of other patients, personnel and property.
- Providing the Hospital with accurate and important information concerning their sources of payment and ability to meet financial obligations.
- The medical consequences if they refuse treatment or do not follow the healthcare provider's instructions.
- Working in partnership with other healthcare providers to develop a pain management plan.
- Provide accurate medication information; prescribed, over the counter, herbals, and any other medications.
