Registration
Pre-Registration:
To greatly reduce your wait time you may pre-register over the phone at your convenience. All of your registration information will be taken over the phone and when you come into the hospital you may go directly to the proper department for your appointment. Any incoming patient for inpatient or outpatient services can pre-register. To pre-register please call Patient Registration at 728-4600 or call toll free 1-800-470-3422.
Registration:
Patient Registration / Admitting is on the immediate right just inside the main entrance. If you have not pre-registered you must check in and wait to speak with an admitting clerk, after which you will be directed or escorted to the proper department for your appointment.
What to Bring for Registration:
- Insurance card
- Driver's license or identification card
- Physician's order if your doctor has given you one (your doctor may fax this order over to the hospital)
To Cancel an Appointment:
To cancel an appointment at the hospital, call the Scheduling Department at 728-4092.
